No, an extractor fan does not need a light, and plenty of bathrooms run the two as entirely separate fittings. But if your bathroom has no opening window, it does need mechanical ventilation by law, and a combined fan/light unit is one popular way to get that ventilation into a small ceiling without cluttering it up. This question lands on our desk a lot, usually from someone in a small en-suite wondering why the electrician is asking about fans when they only came in for a light. We are a lighting retailer, not a ventilation specialist, so what follows is the lighting angle and the ventilation basics, not a full guide to fan specification. Does a windowless bathroom legally need an extractor fan?
Yes. You do not need a window in a bathroom, but if there is not one that opens, the room must have mechanical extract ventilation under Approved Document F. That is the legal minimum, whether you are renovating an existing bathroom or fitting a new en-suite. Steam has to go somewhere; without a window and without a fan, it goes into your ceiling, your grout and eventually your mood.
This is also why so many small en-suites end up with a fan/light combo: a unit that puts a fan and a light into one housing and one ceiling cut-out. Often there is only one sensible spot on the ceiling, ventilation is compulsory, and a light was going there anyway, so combining the two solves a real problem on a low ceiling. It is just not the only way to solve it.

Neither option is always right. Here is the trade-off as we actually see it.

The design point is the one people underestimate. Buy a combined unit and you are choosing whichever light happens to be bolted to the fan, almost always a plain frosted diffuser. If you have found a bathroom light you actually love, a combined unit is rarely the fitting doing the loving. Fan motors also tend to wear out before decorative fittings do, and in a combined unit that often means replacing a perfectly good light along with it. Keep them separate and a fan replacement stays just that.
What IP rating and electrical rules apply to a fan/light combo?
A combined unit still has to respect the same bathroom IP zones as any other fitting: above or near the bath or shower is Zone 1, needing at least IP44. The bit that catches people out is the wiring, not the IP rating. Any new bathroom electrical circuit, combo included, is notifiable work under Part P, so it needs to be installed, or at least signed off, by a registered electrician or competent-person-scheme member. Bathrooms mix water and mains electricity in a small space; this is not a job for a free Sunday with a screwdriver.
Worth knowing before you specify a combined unit: many are wired so the fan keeps running on a timer after the light is switched off, rather than cutting out the second you leave. That overrun is what actually clears the steam, so ask your electrician whether the unit you are looking at has it.
When would KES recommend a combined unit, and when would we keep them separate?
Fit a combined unit in a small en-suite or internal shower room with limited ceiling space, where there was only ever going to be one central fitting. The neatness genuinely wins there, and one cut-out beats two. We would not talk you out of it.
Keep them separate the moment the light fitting is doing design work: a statement flush fitting over a freestanding bath, a pair of wall lights either side of a mirror, anything chosen on purpose rather than accepted because a fan housing allowed it. This is also the right call in most family bathrooms, large enough to treat the fan and the light as two decisions, not one compromise.
